Summit Hospice offers services to bring comfort to those who face the end of their lives and help them reach acceptance and peace of mind. Our interdisciplinary team understands the importance of providing quality care, meaningful education, and helpful resources all with compassion to our patients, their families and caregivers.

Orthopaedic Home Care

Summit Orthopaedic Home Care knows orthopedics. Our clinicians have strong orthopedic training and experience and it’s what they do all day everyday. We understand the importance of creating value to the healthcare system by achieving superior clinical outcomes with high patient satisfaction at a low cost. We’ve been committed to these principles since our founding. It’s in our DNA. We’ve been creating value for years for Preferred Provider Networks, ACO’s, and Bundled Payment Programs including BPCI Advanced and CJR.

Home Care (Assisted Living)

The Assisted Living and Independent Living Division offers dedicated care teams serving the needs of the patient and senior communities they support. Our clinicians don’t simply work in the senior communities, they become part of the community’s team. We offer specialized clinical care. Our therapists and nurses understand the clinical importance of higher levels of acuity seen in Assisted Living Centers. Summit clinicians specialize in geriatric care with an emphasis on dementia and other cognitive related issues. Geriatric care is not just our focus, it’s our passion.

Neurologic Home Care

At Summit Neurologic Home Care, we provide specialized care to our patients in order to promote their independence, which in turn improves their quality of life. Our organization works as a comprehensive and interdisciplinary unit, consistently collaborating and communicating from the moment we receive a referral to discharge and/or transition to the next level of care. Our Neurologic Division operates with 3 clear goals in mind: improve our patient’s independence, decrease caregiver burden, and decrease the rate and risk of readmission.

Infusion Services

Home infusion services encompass many types of infusion therapy which include; intravenous antibiotics, IVIG, TPN(total parenteral nutrition), tube feed, hydration and many more. If you are a patient or a loved one and need to have these services provided please contact our office to discuss your situation. Our infusion team will work with you to discuss your needs and then to make an appropriate referral to an infusion pharmacy of your choice. We can recommend an infusion pharmacy if you have no experience in this area- we pay close attention to your insurance to make sure that the infusion pharmacy you are referred to is an in-network provider for your plan.

Hospice

Summit Hospice knows how privileged we are to be trusted with caring for yourself or your loved one at this stage of life.. Our highly trained hospice team consists of Nurses, Social Workers, State Tested Nurse Aides, Spiritual Care and Bereavement professionals, Volunteers and a Medical Director that can manage your needs in this difficult time. We understand the importance of providing quality care, meaningful education, helpful resources all with compassion to our patients, their families and caregivers.
